Attached are the forms you need to complete for entering your artwork for exhibition in the Northeastern U. Show.

We will NOT have many forms available on Monday, so please PRINT your own forms, have them filled out, and bring them with you to the meeting OR
mail prior to the meeting, they must be postmarked on or by July 15 in order to be entered for the show.
****(You may want to print yourself two copies or make a copy of your entry form for your records and to make sure it matches the card with all your information on the back of your painting).
We have two guest speakers on Monday, so it will help if you have your forms already ready to turn in so we don't disrupt the speakers.
Please PRINT and return completed Entry Form, Release of Liability, and your $10 fee PER ENTRY.....
via postal mail (must be postmarked on or before July 15) to Byrene Culver, 8006 S. 75th East Avenue, Tulsa, OK 74133.
or you can turn them in to Byrene Culver, Treasurer, at the Hardesty Library on Monday, July 15.
****PLEASE NOTE
*Due to some problems with entries at our last show, please read the critical information carefully and submit your entries accordingly. No changes can be made to the entries once turned in; no substitutions for other art work or change of names. All entries must be carefully submitted, with all information printed clearly and no changes can be made after turned in. You cannot submit an entry for one painting and title it ORANGES, and then bring in another painting of birds or apples and want to change the title.
Once your entry forms are received, they then go to be recorded down, then all information sent to someone who creates the labels for your piece. This is a two week process, so last minute changes or entries cannot be accomodated.
